Authorization
"I certify that the facts contained in this application are true and complete to the best of my knowledge and understand that if employed, falsified statements on this application shall be grounds for dismissal.
I authorize the investigation of all statements contained herein and the reference and employers listed above to give you any information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release the company from all liability for any damage that may result from utilization of such information.
I also acknowledge that no representative of the company has the authority to make any employment agreement for a specific duration or to make any agreement that contradicts the previously mentioned terms unless it is put in writing and signed by an authorized company representative.
This authorization does not allow the release or use of disability-related or medical information in a manner that violates the Americans with Disabilities Act (ADA) and other relevant federal and state laws.
I acknowledge that the company may need to conduct a consumer credit report or criminal records check before hiring me. If these reports are necessary, I understand that I will receive a written notice as required by federal law, and I will also provide a separate written authorization for these reports. I also understand that having a poor credit history or a criminal conviction will not automatically disqualify me from employment.
As required by federal law, all individuals who are hired will need to verify their identity and eligibility to work in the United States and complete the necessary employment verification document upon being hired.
